Leeds City Schools Returning Student Residency Verification

2024-2025 School Year
Please use the following form to submit proof of residence for the 2024-2025 school year. Please be sure that the name of the parent/guardian, address, and dates are fully visible or you will be asked to resubmit. All submitted documents should be dated for June or July of the current year. All families are required to provide TWO residence documents from the list below. You may submit two utility bills or a mixture of utilities and lease/mortgage.

You may submit one form for all of your children provided you have 8 or fewer children in our school system. If you have more than 8 children in our system, please submit a second form.
1. Current Utility Bills (Disconnect Notices are Not Allowed). Utility bills or receipts of the following type that show the name, address, and current date are acceptable: Alabama Power, Leeds Water Works, Jefferson County Sewer, or Spire Gas. If your lease includes any of these items, please highlight this within the lease that you submit.

2. Current lease agreement that lists the address of the property and all occupants, current mortgage statement, or property deed.

**If you do not rent or own your residence or if you live with someone else and cannot produce the required items, you will need to complete the Third Party Application. Please contact Gina Ragan at gragan@leedsk12.org at Leeds City Schools Board of Education to make an appointment. All third-party residency applications will be handled at Leeds City Schools Board of Education. The phone number is 205-699-KIDS (5437) and the Board Office is located at 1517 Hurst Avenue.**

If your child will be a new student for the 2024-2025 school year, please contact your child's school to schedule an enrollment appointment. New students will not be required to provide residency documentation through this form. They will provide this in person during the enrollment appointment.
